Nursing 250: Exploration of Professional Nursing Practice is the first nursing course in the Post LPN BN program. The course is developed to honor the students' achievements in the Licensed Practical Nurse (LPN) role and to introduce the concepts of professional Registered Nurse (RN) practice to the LPNs enrolled in the Post LPN BN Program. Topics addressed in the course include transition to the Graduate prepared RN role, use of scholarly evidence to support professional nursing practice, introduction to family, community, ethics, legal foundations, and cultural concepts in professional nursing practice.

After completing Nursing 250: Exploration of Professional Nursing Practice , you will be able to do the following:

  • Describe and analyze the effect of personal values, beliefs, and assumptions on the development of one's own professional identity and self-concept as a Graduate prepared RN.
  • Demonstrate awareness of the concepts of role transition and change throughout the journey from LPN to Graduate prepared RN.
  • Identify the effect of significant historical events on the development of professional nursing.
  • Articulate the role changes of the professional nurse as the focus changes from illness to health promotion and care shifts from the individual to families, groups, and communities.
  • Discuss how cultural diversity is pertinent to the care of individuals, families, and communities.
  • Articulate similarities and differences between the LPN to Graduate prepared RN in relation to the ethical and legal foundations for professional nursing practice as outlined in the code of ethics, professional competencies, and the standards of nursing practice.
  • Distinguish credible sources of literature, conduct advanced library searches, and present all work in current APA format.

To receive credit for NURS 250, students must complete and submit all of the required assignments and achieve an overall composite grade of at least C (64 percent) . All papers submitted must follow the current addition of the APA Publication Manual .

Acute Care Patient Case Study

This paper is a case study that focuses on the clinical management of a patient with cirrhosis of the liver, along with other complications like ascites, hyperammonemia, bowel obstruction, and infection. It provides an overview of the disease process associated with the patient's condition, the pathological alterations that take place in the liver during the development of cirrhosis, and the contributing factors that lead to the progression of cirrhosis. Additionally, the paper discusses approaches to medical and surgical therapy, pharmacological interventions, and nutritional concerns. It highlights evidence-based teaching materials, collaboration, research studies, and patient education, as well as self-care, health promotion, illness prevention, and management patient education.

NURS 333: Adult Health II Case Study

Purpose of Assignment: The purpose of this case study is to integrate knowledge from the humanities and sciences, including nursing research and theory, to plan, provide, and evaluate holistic care provided to a client selected during this clinical rotation.

Student Approach to Assignment: I approach this assignment with a focus on the case study I completed during my Adult Health II course. Through the case study, I have delved into my comprehension of the disease process linked to the patient's alterations. The paper explores the pathological changes that occur in the liver during the progression of cirrhosis. It discusses various methods of medical and surgical treatment, as well as the use of medications and considerations related to nutrition. In this case study, I have applied standards of practice, institutional protocols, and safety and quality standards backed by research studies in the healthcare field. 

Reason for Inclusion of this Assignment in the Portfolio: This assignment highlights my proficiency in clinical management and nursing interventions for patients with this intricate condition. The paper illustrates my in-depth understanding of the disease process and its various aspects, including medical and surgical treatments, pharmacological interventions, nutritional considerations, patient education, and health promotion. This further emphasizes my proficiency in utilizing evidence-based teaching materials, research studies, and institutional protocols in my clinical practice, showcasing my dedication to upholding safety and quality standards. This assignment showcases my expertise in clinical practice, nursing theory, and interventions, making it an excellent addition to the portfolio.

Domain 1: Knowledge for Nursing Practice

In the paper, I explained in the study why the patient is at a greater risk for infection. The presence of a central line catheter, wounds, bacterial peritonitis, and the consequence of ascites all contribute to an elevated risk of infection. The prompt administration of antibiotics and strict adherence to the CLABSI protocol were implemented. The catheter site was evaluated and maintained by following proper hand hygiene procedures, wearing gloves, cleaning the catheter hub with alcohol wipes to avoid bacterial transmission, ensuring the dressing around the insertion site was secure and clean, and using disinfecting green caps.

Domain 2: Person-Centered Care

In the case study, I employ outcome measures to assess the efficacy of care. For example, the outcome identification for the nursing diagnosis of imbalanced nutrition involves patients attaining an improved nutritional status by gaining weight and exhibiting elevated indicators of malnutrition. The patient has not observed weight gain, although the patient tolerates the required calorie intake as prescribed by the certified dietician. At the end of my shift, the patient was being transitioned from total parenteral nutrition (TPN) to a regular adult diet, which included a high-fiber component as advised by qualified dieticians. The patient can tolerate the dietary modifications, but there is a significant increase in the volume of stool collected in the ostomy bag. I recommended decreasing the lactulose dosage to allow the stomach to process and absorb the nutrients effectively.

(13). Evaluates nursing care outcomes through the acquisition of data and the questioning of inconsistencies. (Domains 2, 3)

I evaluated patient assessment data, labs, and clinician notes in the case study to assess patient outcomes. Nursing care focuses on monitoring the patient's nutritional status and respiratory function. The patient received total parenteral nutrition (TPN), a form of intravenous feeding containing significant sugar. Combined with the body's natural stress response, it can elevate blood glucose levels and increase the risk of hyperglycemia. Monitoring other electrolytes, such as potassium, is essential as insulin utilization can lower potassium levels and harm the patient's cardiac health, potentially leading to PVC. Additional gastrointestinal complications may arise in conjunction with TPN. The patient's need for additional nourishment underscores the significance of monitoring specific laboratory tests to safeguard the patient's overall health and safety.

In this paper, I discuss alcoholic cirrhosis of the liver, which is caused by chronic inflammation leading to damage and replacement of liver cells with fibrous tissue. Prolonged and excessive consumption of alcohol, viral infections, and accumulation of fat in the liver are common causes. Complications include jaundice, portal hypertension, ascites, esophageal varices, alterations of coagulation factors, splenomegaly, hepatic encephalopathy, alteration of the skin, and malnutrition. Cirrhosis is a major cause of death globally, primarily due to alcohol consumption and nonalcoholic fatty liver disorders. The study emphasizes the importance of appropriate knowledge of significant health problems to guide my nursing practice in such cases.

The case study explores the sources and applications of national safety and quality standards and hospital standards of practice to provide guidance for nursing practice. During the patient's withdrawal process, we prioritized their safety and well-being by implementing the CIWA protocol and seizure precautions. This was done to address alcohol dependency and withdrawal symptoms. The CIWA protocol, commonly known as the Clinical Institute Withdrawal Assessment, is a valuable tool in the monitoring and management of symptoms related to alcohol withdrawal. It helps healthcare professionals keep a close eye on symptoms like tremors, anxiety, and hallucinations. Seizure precaution involves using strategies to prevent and effectively handle seizures, which may arise in individuals experiencing severe alcohol withdrawal. It is essential to take certain precautions to ensure the safety and wellbeing of the individual. This involves removing any objects that may pose a risk, providing cushioning to prevent potential injuries, positioning them on their side to prevent choking, and contacting medical professionals if needed.
