How to Talk to Your Family About Their Heart Health History

H ypertrophic obstructive cardiomyopathy (HOCM) is the most common genetic heart disease, affecting about 1 in every 500 people, according to the American Heart Association (AHA). In people with HOCM , genetic variants cause the heart’s walls to thicken and stiffen, blocking blood from flowing freely from the left ventricle to the aorta. This, in turn, results in shortness of breath and chest pain (especially during physical activity), abnormal heart rhythms, lightheadedness, dizziness, and fainting, and can worsen over time.

If a parent has HOCM, offspring have a 50% chance of inheriting it. That means knowing your family’s heart health history is crucial: If your doctor is aware that you have relatives with HOCM, they can “screen family members early on, before they get sick or have any cardiac complications” using EKG and echocardiogram, says Dr. Ali Nsair, co-director of the Hypertrophic Cardiomyopathy Clinic at UCLA Health.

About 60% of the time, genetic testing can identify a specific change in a gene that causes HOCM. Even if you (or your kids) test negative for the particular genetic variant your parent with HOCM has, you can still be screened every few years with EKGs, echocardiograms, and visits to a cardiologist to make sure complications haven’t popped up, Nsair says.

And it’s not only HOCM that can cluster in families. “A lot of what ails us is in some sense heritable,” says Dr. Daniele Massera, associate director of the Hypertrophic Cardiomyopathy Program at NYU Langone Health. “Whatever affects your family members might directly affect you.” Other heart conditions, like familial hypercholesterolemia (high cholesterol) and high lipoprotein (a) (proteins and fats that carry cholesterol), can be inherited, and a family history of heart disease that isn’t genetic puts you at higher risk, too.

But no single risk factor—including genetics—is a guarantee that heart disease will develop down the line: “For me, the most important reason to know your family history is prevention,” says Dr. Svati Shah, a member of the American Heart Association’s National Board of Directors and director of the Duke Adult Cardiovascular Genetics Clinic. If you know you have an increased risk for heart disease due to your genes or family history, which you can’t control, you can take heart-healthy steps to improve the lifestyle factors you can control, such as getting plenty of sleep, eating a balanced diet, and staying active, according to the AHA.

To make sure you get access to the testing, treatment, and information on lifestyle changes that can help you avoid or delay inherited heart health complications, it’s important to stay on top of your family’s medical history. Here’s how to have those conversations with honesty and compassion while still getting the potentially life-saving answers you need.

Start with broad questions

You might open the conversation with a question as simple as “Do you have any kind of heart disease?” or as general as: “Have you ever had any chest pain?” Shah suggests.

If your relative isn’t entirely sure about their diagnosis or past procedures, consider asking if a doctor has ever told them they had any of the following, according to the Centers for Disease Control and Prevention (CDC):

  • Coronary artery disease or atherosclerosis
  • Heart attack
  • Atrial fibrillation
  • Cardiomyopathy
  • Heart failure
  • Aortic aneurysm

Also ask if they have a pacemaker or have ever had heart bypass surgery. If they’ve given birth, Shah suggests adding: “Did anything happen [to your heart] when you had your babies? Did you get really high blood pressure?” And whenever possible, ask what age they were when they experienced these conditions or complications for the first time, according to the CDC.

The details might get fuzzier as you go back generations. “Often people say [things like], ‘My dad died at 47 from a heart attack,’ but it’s actually that they didn’t wake up from sleep, and it may not have been a heart attack,” Massera says.

Try to get as many details as you can, because those specifics can help your doctor determine the best next steps for you. For example, you might need different testing if your 47-year-old father died of sudden cardiac arrest (when the heart suddenly stops beating) rather than a heart attack (when an artery to the heart is blocked). “To distinguish between the two is really critical: A heart attack is common, but if we identify sudden cardiac death as the real mechanism, then we’re homing in on a more narrow group of conditions that will require testing that you wouldn’t necessarily do if you’re talking about a heart attack,” Massera says.

While heart attacks , strokes, and sudden cardiac death might stand out the most in your relatives’ memories, make sure to ask about heart disease risk factors too, like high blood pressure, high cholesterol, and diabetes. “There is a strong predictor among those factors that can lead to heart disease and heart failure,” Massera says.

Talk to three generations on both sides of your family

Ideally, aim to include three generations on both sides of your family in your discussions about heart health: your grandparents, your parents and their siblings, and your siblings.

“Backwards more than three generations, people don’t really know what happened to those relatives,” Shah says. But any information you can collect is still better than nothing, especially if you continue to gather knowledge over time. “[Learn] as much as you can, and it can be over the course of many years that you fill in the details,” she says.

If you or your siblings have children, note any known heart health information about them, too, per the CDC.

These discussions may not go as well if your brother feels interrogated or your mother feels blamed. “These can be really laden conversations,” Shah says. “Especially when you start talking about weight, high cholesterol, blood pressure—people can get sensitive about that.”

If a family member remains standoffish, don’t press: “If that person isn’t ready, it’s OK, circle back to it,” Shah says. Your relatives might feel more comfortable in a group setting. “Sometimes one on one, people ask: ‘Why are you calling me ? Why are you worried about my health? Why aren’t you worried about other people’s health?’” Group conversations have the added benefit of helping to nudge everyone’s memory in the right direction, too. “Sometimes one person remembers one thing, another person remembers another thing, but if you spoke with each one independently, you wouldn’t have made the connection,” Shah says.

These conversations don’t have to be done in person, but face-to-face discussions allow you to pick up on a relative’s body language more easily and change the subject if you can tell they’re uncomfortable.

Record the information somewhere you can access it easily

You can use digital tools like the Surgeon General’s My Family Health Portrait or the Global Alliance for Genomics and Health’s Family History Toolkit to record and store your family’s heart health history.

Don’t feel pressured to use software: Typing notes into your smartphone or jotting them down on paper is fine, too. As long as it’s a system that works for you and you know where the information is, you’ll be less likely to forget any details when you’re actually sitting in front of your doctor.

“I love it when patients come in with a printout,” Massera says. He makes sure to devote plenty of time to walk through all of a patient’s relatives and their relevant health history, but recognizes a typical primary care doctor might not have that luxury. “You can’t do this if you see a patient in five minutes,” he says. If you feel like your doctor isn’t giving you enough time to cover your family history thoroughly, it’s OK to ask for a longer appointment to address your concerns, he adds.

Report back to your doctor

Simply knowing your family’s heart health history isn’t enough to prevent your own heart issues. Sharing what you’ve learned with your doctor is key to determining the screenings, treatment, or lifestyle changes that might benefit you.

To that end, share “broadly” with your primary care doctor once you’ve asked your family about their heart health, Nsair says. Your doctor will dig deeper into the information that’s most relevant to your individual health, but it’s always better to provide too much than too little.

A history of heart failure, heart rhythm disorders , stroke, and sudden death, especially in relatives younger than 40 or 50, will likely prompt your primary care doctor to refer you to a cardiologist. That person or your primary care doctor can help you identify modifiable risk factors that you can change, such as quitting smoking, adopting a balanced diet, starting an exercise routine, and maintaining a healthy weight.

You won’t have to do this every time you visit the doctor: Once you’ve shared your family heart health history, that information is entered into your medical records, so anyone who is a part of your care team will have access to the same details.

Chat again whenever big changes occur

Your family’s heart health will continue to change over time—after all, many heart issues, including HOCM, are more common in middle age—so it’s hard to say exactly how often to ask your relatives about their heart health.

In general, it’s a good idea to collect more information whenever a family member experiences a major heart-related health issue, like a sudden death, cardiac arrest, or having a defibrillator implanted. “This is not a conversation you need to have every year. But every few years, reassess,” Shah advises.

Remember, these conversations may be challenging, but they’re empowering you with the information you need to live well for longer. “Genetics is not destiny. There’s a saying that genetics loads the gun, but the environment pulls the trigger,” Shah says. “You have control over this.”

Storms in Tampa Bay lead to watches, warnings and so far leave minor damage

Severe weather rolled into the Tampa Bay area Thursday afternoon, leading to a series of weather watches and warnings, and leaving — as of about 3 p.m. — only minor damage behind.

Initial damage assessments were still under way, but early reports are that a funnel cloud was spotted in New Port Richey, winds gusted to 60 mph in some areas, at least two homes in Pinellas County were damaged by lightning, and some flooding was reported in northern areas of Tampa Bay.

Meanwhile, coastal flooding was still a concern Thursday afternoon as high tide neared in some areas.

Forecasters had placed all of Tampa Bay under a “slight” risk of severe weather Thursday. A slight risk is the second level on a scale of 1 to 5, with “marginal” being the lowest chance of severe weather and “high” being the greatest chance, according to the weather service’s Storm Prediction Center.

The harsh rain and winds stemmed from a weather system that battered parts of the South on Wednesday from Texas to the Florida Panhandle.

The weather service on Thursday morning placed the Tampa Bay area and much of north central Florida under a tornado watch until 3 p.m., which meant conditions were favorable for tornadoes to develop. A tornado warning, of which several were issued across the area Thursday, means a tornado has been sighted or there is an indication of one on a weather radar.

Portions of Florida’s west coast, including coastal areas of Tampa Bay, are under a coastal flood warning until 8 p.m.

3 p.m. A survey of damage

Reports of damage, while still being assessed, were minimal across Tampa Bay, according to the National Weather Service.

Several areas were under a tornado watch Thursday afternoon, though there were no official reports of a tornado. However, a funnel cloud was spotted around 12:30 p.m. in New Port Richey, according to the weather service.

Gusty winds also blew through the area Thursday afternoon, with winds nearing 60 mph at Tampa International Airport.

In Pinellas County, dispatchers received 911 calls — one in St. Petersburg and another in Treasure Island — for residential fires caused by lightning strikes, according to the weather service.

In Clearwater, water levels were falling. A tidal gauge in Clearwater showed that water levels had peaked at 2.6 feet around 12:30 p.m., just near minor flooding.

In St. Petersburg, water levels were approaching high tide. Around 3 p.m. water levels were nearing 2 feet. Across the Bay in Tampa, water levels were also nearing high tide, which was set for around 4 p.m. and the tide was reaching around 2 feet.

Minor flooding in St. Petersburg and Tampa occur at slightly above 2 1/2 feet.

2:45 p.m. Tarpon Springs flooded

Around the area, the weather service reported flooding at the Fred Howard Park parking lot, a beach in Tarpon Springs. Just a few miles north near U.S. 19 North and Green Key Road in New Port Richey, flooding reached up to a foot, according to the weather service.

1:35 p.m. Flooding at Hudson Beach

Spectrum Bay News 9 reporter Calvin Lewis posted to social media video of Hudson Beach flooded. Lewis reported that a few streets nearby were also under water.

1:26 p.m. Large tornado spotted in St. Augustine

Matt Devitt, a meteorologist from South Florida, posted a video sent to him showing a large tornado in the World Golf Village area of St. Augustine.

While a number of tornado warnings were in place for areas in Tampa Bay, there have been no official reports of tornadoes.

1:21 p.m. Outages across Tampa Bay

As gusty winds and tornado warnings tore were issued for areas of Tampa Bay Thursday afternoon, electric companies began reporting outages.

Tampa Electric Co. reported more than 3,000 customers were without power in Hillsborough and portions of Pasco County. At least 1,000 customers were without power in the Temple Terrace area.

A power outage map from Duke Energy showed more than 1,000 customers had no power. A large portion was concentrated in the New Port Richey and Port Richey areas of Pasco County.

1:26 p.m. Blustering winds in Pinellas County

Video feed from Treasure Island, a coastal town in Pinellas County, showed swirling winds and thick sheets of rain in portions of the town just before 1 p.m.

Tampa International Airport recorded wind gust up to 43 mph at 1 p.m., according to the National Weather Service.

12: 58 p.m. Tornado warning for Temple Terrace

The National Weather Service issued a tornado warning for Temple Terrace, an unincorporated area of Hillsborough County, until 1:15 p.m.

12:56 p.m. A look at tides

Coastal areas of Tampa Bay are under a coastal flood warning until 8 p.m., when forecasters said up to 4 feet of flooding is possible.

Rick Davis, a meteorologist with the weather service’s Tampa Bay office told the Tampa Bay Times on Wednesday that should storms arrive at lower tide, flooding may not be as bad. If storms line up with high tide later Thursday afternoon, flooding could be worse.

In Clearwater, the next highest tide is expected at just after 1 p.m. Observations from the National Oceanic and Atmospheric Administration show water levels had reached just under 2 1/2 feet before 1 p.m.

I n St. Petersburg, high tide is set for just before 4 p.m . Projected tides from the National Oceanic and Atmospheric Administration show tides peaking at just above 2 1/2 feet. The current water levels were slightly above 1 foot.

12:49 p.m. Tornado warning for portions of Hillsborough County

The National Weather Service showed a possible tornado approaching near Citrus Park and Tampa. The warning remains in effect until 1 p.m.

12:45 p.m. Whipping winds

The National Weather Service told the Tampa Bay Times on Wednesday that wind gusts up to 70 mph were possible with the severe weather.

According to Spectrum Bay News 9 , its Klystron 9 radar showed strong winds in Pinellas County, up to 60 mph, were possible. In Seminole, the line of storms blew winds up to 52 mph, Bay News 9 reported.

At Tampa International Airport, gusts of up to 43 mph were observed just after noon, according to the weather service.

12:17 Northern Pinellas under tornado warning

Much of northern Pinellas, from Indian Rocks Beach up to Palm Harbor, are under a tornado warning until 12:45 p.m.

7.4 magnitude quake hits Taiwan, strongest in 25 years

Rescuers are working to free dozens trapped after a 7.4 magnitude earthquake struck the east coast of Taiwan — causing landslides and collapsed structures.

At least nine people have died , more than 900 others are injured and over 100 buildings have been damaged.

The quake is the strongest to hit Taiwan in 25 years .

Here's what else we know:

  • The quake:  The earthquake  hit  at 7:58 a.m. local time, 18 kilometers (11 miles) south of the city of Hualien at a depth of 34.8 kilometers, according to the US Geological Survey.
  • Aftershocks : The quake was followed by 29 aftershocks greater than a magnitude of 4.0 near the epicenter of the earthquake in east Taiwan so far, according to the United States Geological Survey (USGS). Tremors have been felt across the island, including in Taipei. Tremors as high as magnitude 7 are  expected  in the following days.
  • Hualien County:  The region where the quake struck, Hualien County, has a population of about 300,000, around 100,000 of whom live in the main city of Hualien. But many in the region live in remote coastal or mountain communities that  can be hard to reach , so it might take time to understand the extent of Wednesday's quake.
  • Trapped miners: Taiwan's national fire agency said that 71 people are trapped in two mines in Hualien.
  • Power cuts : More than 91,000 households were without electricity, according to Taiwan's Central Emergency Command Center.
  • Medical facilities: Hospitals across Taiwan’s capital , Taipei City, are operating normally despite being damaged by Wednesday’s earthquake, according to the Municipal Government.
  • US monitoring: The Biden administration is monitoring the earthquake in Taiwan overnight and is prepared to offer assistance, a National Security Council spokesperson said Wednesday. 

71 miners trapped in 2 mines in Taiwan after earthquake, national fire agency says

Taiwan's national fire agency said that 71 people are trapped in two mines in Hualien after a powerful earthquake struck the island. 

In the Heping mine, there are 64 people trapped, and seven more are trapped in the Zhonghe mine, the fire agency said in a news conference on Wednesday.

Taiwanese semiconductor facilities will resume production overnight following earthquake

Taiwan Semiconductor Manufacturing Company Limited (TSMC), the chipmaking giant, said on Wednesday that its facilities which were impacted by the 7.4 magnitude earthquake are expected to resume production overnight. 

TSMC reported that their overall tool recovery is at more than 70% within 10 hours of the earthquake striking the island. Safety systems are also operating normally, TSMC added.

The company noted that a small number of tools were damaged but that there was no damage to its extreme ultraviolet lithography (EUV) tools. Work at construction sites will resume after further inspections are complete, TSMC said.

Earlier, a TSMC spokesperson told CNN they had evacuated some manufacturing plants. All personnel are now safe, TSMC said in an update.

Biden administration monitoring Taiwan earthquake, White House says

The Biden administration is monitoring the earthquake in Taiwan overnight and is prepared to offer assistance, a National Security Council spokesperson said Wednesday. 

"We are monitoring reports of the earthquake impacting Taiwan and continue to monitor its potential impact on Japan. The United States stands ready to provide any necessary assistance. All those affected are in our prayers," a statement from National Security Council spokesperson Adrienne Watson said.

7 major earthquakes have hit Taiwan over the last 50 years

The 7.4 magnitude earthquake that killed at least nine people and injured hundreds Wednesday, is the strongest to hit Taiwan in 25 years .

Over the last 50 years, the island has experienced a total of seven major earthquakes, the last being a 7.1 magnitude quake in 2006 in Pingtung County in southern Taiwan.

The island sits on the Pacific Ring of Fire , which makes it prone to earthquakes.

There have been 29 aftershocks greater than a magnitude of 4.0 near the epicenter of the earthquake in east Taiwan so far, according to the United States Geological Survey (USGS).

Of these aftershocks:

  • One was above 6.0
  • 13 have been at or above 5.0
  • 14 have been above 4.0.

Forecast during recovery efforts: Tonight's forecast in Hualien City, near the epicenter, calls for increasing cloud coverage. Thursday is looking at mostly cloudy skies with afternoon showers and rain Thursday night and Friday during the day.

Rainfall totals should be relatively light for Taiwan, with models calling for under 25 mm (less than 1 inch) by Friday evening local time.

A dashcam camera has caught the moment a large landslide came down a mountain in Taiwan, triggered by a 7.4 magnitude earthquake on Wednesday morning.

The quake is the strongest to have rattled the island in 25 years, killing at least nine people and leaving more than 150 trapped.

Why is looking at a solar eclipse dangerous without special glasses? Eye doctors explain.

The solar eclipse will be visible for millions of Americans on April 8, 2024, making many excited to see it — but how you watch it matters, since it can be dangerous for your eyes. 

A  solar eclipse occurs when the moon passes between the sun and Earth, blocking the sun's light . When the moon blocks some of the sun, it's a partial solar eclipse, but when moon lines up with the sun, blocking all of its light, a total solar eclipse occurs,  NASA explains . Either way, you need eye protection when viewing.

"The solar eclipse will be beautiful, so I hope that everyone experiences it — but they need to experience it in the right way," said Dr. Jason P. Brinton, an ophthalmologist and medical director at Brinton Vision in St. Louis.

Here's what to know to stay safe.

Why is looking at a solar eclipse dangerous?

Looking at the sun — even when it's partially covered like during an eclipse — can cause eye damage.

There is no safe dose of solar ultraviolet rays or infrared radiation, said  Dr. Yehia Hashad , an ophthalmologist, retinal specialist and the chief medical officer at eye health company Bausch + Lomb.

"A very small dose could cause harm to some people," he said. "That's why we say the partial eclipse could also be damaging. And that's why we protect our eyes with the partial as well as with the full sun."

Some say that during a total eclipse, it's safe to view the brief period time when the moon completely blocks the sun without eye protection. But experts warn against it. 

"Totality of the eclipse lasts only about 1 to 3 minutes based on geographic location, and bright sunlight suddenly can appear as the moon continues to move," notes an eclipse viewing guide published in JAMA , adding, "even a few seconds of viewing the sun during an eclipse" can temporarily or permanently damage your vision. 

Do I need special glasses for eclipse viewing?

Yes.  Eclipse glasses are needed to protect your eyes if you want to look at the eclipse.

Regular sunglasses aren't protective enough for eclipse viewing — even if you stack more than one. 

"There's no amount of sunglasses that people can put on that will make up for the filtering that the ISO standard filters and the eclipse glasses provide," Brinton said.

You also shouldn't look at the eclipse through a camera lens, phone, binoculars or telescope, according to NASA, even while wearing eclipse glasses. The solar rays can burn through the lens and cause serious eye injury.

Eclipse glasses must comply with the  ISO 12312-2 international safety standard , according to NASA, and should have an "ISO" label printed on them to show they comply. The American Astronomical Society  has a list  of approved solar viewers.

Can't find these, or they're sold out near you? You can also  make homemade viewers ,   which allow you to observe the eclipse indirectly — just don't accidentally look at the sun while using one.

How to keep kids safe during the solar eclipse

Since this eclipse is expected to occur around the time of dismissal for many schools across the country, it may be tempting for students to view it without the proper safety precautions while getting to and from their buses. That's why some school districts are  canceling classes early so kids can enjoy the event safely with their families.

Dr. Avnish Deobhakta, vitreoretinal surgeon at New York Eye and Ear Infirmary at Mount Sinai, said parents should also be careful because it can be difficult for children to listen or keep solar eclipse glasses on. 

"You want to actually, in my opinion, kind of avoid them even looking at the eclipse, if possible," he said. "Never look directly at the sun, always wear the right eclipse sunglasses if you are going to look at the sun and make sure that those are coming from a reliable source."

Brinton recommends everyone starts their eclipse "viewing" early, by looking at professional photos and videos of an eclipse online or visiting a local planetarium. 

That way, you "have an idea of what to expect," he said. 

He also recommends the foundation  Prevent Blindness , which has resources for families about eclipse safety.

What happens if you look at a solar eclipse without eclipse glasses?

While your eyes likely won't hurt in the moment if you look at the eclipse without protection, due to lowered brightness and where damage occurs in the eye, beware: The rays can still cause damage .

The harm may not be apparent immediately. Sometimes trouble starts to appear one to a few days following the event. It could affect just one or both eyes.

And while some will regain normal visual function, sometimes the damage is permanent. 

"Often there will be some recovery of the vision in the first few months after it, but sometimes there is no recovery and sometimes there's a degree to which it is permanent," Brinton said. 

How long do you have to look at the eclipse to damage your eyes?

Any amount of time looking at the eclipse without protection is too long, experts say. 

"If someone briefly looks at the eclipse, if it's extremely brief, in some cases there won't be damage. But damage can happen even within a fraction of a second in some cases," Brinton said. He said he's had patients who have suffered from solar retinopathy, the official name for the condition.

Deobhakta treated a patient who watched the 2017 solar eclipse for 20 seconds without proper eye protection. She now has permanent damage in the shape of a crescent that interferes with her vision. 

"The crescent that is burned into the retina, the patient sees as black in her visual field," he said. "The visual deficit that she has will never go away."

How to know if you've damaged your eyes from looking at the eclipse

Signs and symptoms of eye damage following an eclipse viewing include headaches, blurred vision, dark spots, changes to how you see color, lines and shapes. 

Unfortunately, there isn't a treatment for solar retinopathy.

"Seeing an eye care professional to solidify the diagnosis and for education I think is reasonable," Brinton said, but added, "right now there is nothing that we do for this. Just wait and give it time and the body does tend to heal up a measure of it."
